I noticed my self getting stressed out lately and it made sense to me to go see a therapist. Especially since it is part of my health insurance and its not like I'm losing anything in the process. So to get ready I found this set of pretherapy questions I should answer before I go
1. Have you had prior psychotherapy? If so, when?
yeah, from age 6-7 to age 13-14 and then for a little bit when I was 21
2. Are you currently on any medication?
lithium
3. Are you allergic to any medication? If so, which ones?
n/a
4. Do you have a family history of mental illness or substance abuse?
Father has bipolar, grandmother was alcoholic
5. Have you ever attempted suicide, or had a plan to hurt yourself?
no, don't really have a plan either, though if I was going to do it I would find a way to really fuck my shit up.
6. Do you have any current or past major medical problems? Please describe.
bipolar, adhd, acl replacement surgery. dramatic, trivial, iverson-esque
7. Do you have: 1) current sleep difficulty; or 2) decrease/increase in appetite?
not really
8. What would you like to have happen as a result of this session?
easy goal: Not go to the hospital this school year
medium goal: be able to prepare for school without the feeling of innescapable panic
strech goal Find my lifes purpose, Become teacher of the year, find true love, develop innerpeace and a deep understanding of myself and how to locate this image of myself in regards to race, gender, my family, and the world we live in...and perhaps get laid more regularly
9. Describe your current usage of alcohol/drugs.
lots of alcohol see "Old School" for examples.
